Childhood Apraxia of Speech Explained
CAS is a motor planning disorder, not a phonological one — and the treatments do not transfer. Here's how to tell them apart.

Childhood apraxia of speech (CAS) is a pediatric motor speech disorder in which the precision and consistency of speech movements are impaired in the absence ofneuromuscular deficits. The child knows the word and has adequate strength and tone — the breakdown is in planning and programming the movement sequence.
The ASHA three-feature consensus
ASHA's technical report identifies three features consistent with a deficit in motor planning and programming. These are the features exam items key on:
- Inconsistent errors on consonants and vowels across repeated productions of the same word.
- Lengthened and disrupted coarticulatory transitions between sounds and syllables.
- Inappropriate prosody, especially in the realization of lexical or phrasal stress — often equal stress on all syllables.
Other clinical markers
- Groping and silent posturing before initiating speech.
- Errors increase as word length and phonetic complexity increase.
- Vowel distortions — a strong differentiator from phonological disorder.
- Better production of automatic/overlearned speech than volitional speech.
- Limited consonant and vowel inventory; reduced babbling history.
- Receptive language notably stronger than expressive.
Differential diagnosis
Phonological disorder: errors are rule-governed and consistent (all final consonants deleted); stimulability is often better; prosody is normal. Dysarthria: weakness, hypotonia, drooling, and consistent distortions present even at rest and in non-speech tasks. Severe phonological disorder plus CAS commonly co-occur, and diagnosis may need to be provisional in children under three. Compare against phonological processes before committing to a CAS diagnosis.
Assessment
There is no single confirmatory test. A defensible CAS workup includes an oral mechanism exam (to rule out structural or neuromuscular cause), a phonetic inventory, repeated trials of the same words to document inconsistency, multisyllabic word production, a diadochokinetic task, and connected speech sampling for prosody. Formal tools include the Dynamic Evaluation of Motor Speech Skill (DEMSS), the Kaufman Speech Praxis Test, and the Madison Speech Assessment Protocol. Always evaluate receptive and expressive language, because language impairment and later literacy difficulty frequently accompany CAS.
Treatment: motor learning, not phonology
CAS treatment applies the principles of motor learning. High-frequency, distributed practice with many trials per session beats long, infrequent sessions. Key parameters:
- Blocked then random practice — blocked builds acquisition, random builds retention and transfer.
- Constant then variable practice — vary the target once accuracy stabilizes.
- Reduce feedback frequency over time and shift from knowledge of performance to knowledge of results to promote self-monitoring.
- Use multimodal cueing — tactile, gestural, and visual — and fade systematically.
Named approaches to know: DTTC (Dynamic Temporal and Tactile Cueing), which moves along a cueing hierarchy from simultaneous production to spontaneous production; ReST (Rapid Syllable Transition Treatment) for prosody and transitions in school-age children; PROMPT, a tactile-kinesthetic approach; and integral stimulation ("watch me, listen to me, do what I do"). Non-speech oral motor exercises are not supported for CAS and are a reliable wrong answer on exam items.
AAC and CAS
Augmentative and alternative communication is not a last resort and does not suppress speech development. For children with severe CAS and limited functional output, AAC should be introduced early as a multimodal support while speech treatment continues.
Acquired apraxia of speech in adults
Adult AOS most often follows left frontal lesions, particularly the left insula and premotor cortex, and commonly co-occurs with Broca's aphasia. The signature is slow rate, distorted substitutions, groping, and errors that increase with word length. Treatment emphasizes articulatory-kinematic approaches (Sound Production Treatment) and, for severe nonfluent presentations, Melodic Intonation Therapy.
