Right Hemisphere Disorder & Cognitive-Communication
RHD leaves the linguistic core intact and breaks everything wrapped around it: attention, pragmatics, prosody, and awareness.

Right hemisphere damage (RHD) does not typically impair the linguistic core — syntax, phonology, and basic semantics are usually intact. What breaks is everything wrapped around language: attention, pragmatics, prosody, inference, and awareness. That is why RHD is classified as a cognitive-communication disorder rather than an aphasia.
The core deficit clusters
Left neglect (hemispatial inattention)
Failure to attend to stimuli on the left side of space or the body, most often after right parietal damage. It is an attention deficit, not a visual field cut — though hemianopsia can coexist. Signs: reading only the right half of a page, eating from one side of the plate, shaving one side of the face, drawing incomplete clock faces. Assessment uses line bisection, cancellation tasks, and copying/drawing. Treatment includes visual scanning training with an anchoring cue at the left margin, limb activation, and prism adaptation.
Anosognosia and reduced awareness
Impaired awareness of deficits is one of the strongest predictors of poor functional outcome, because a patient who does not perceive a problem will not use a strategy. Treatment relies on structured self-prediction, performance, and review cycles rather than verbal explanation.
Pragmatic and discourse deficits
Reduced eye contact, poor turn-taking, tangential and verbose output, difficulty reading conversational partners, and impaired topic maintenance. Discourse is often full of irrelevant detail with the main point never stated — the inverse of the "empty but fluent" pattern in Wernicke's aphasia.
Aprosodia
Flat, monotone output (expressive aprosodia) and/or failure to interpret emotional tone in others (receptive aprosodia). Patients may be perceived as depressed or indifferent when they are not.
Inference, figurative language, and humor
Literal interpretation of idioms, sarcasm, and metaphor; difficulty drawing inferences and integrating context to build a coherent situation model. This is a classic RHD test item: the patient understands each sentence but misses the point of the story.
Cognitive-communication after TBI
TBI overlaps with RHD but adds a frontal/executive profile: impaired attention (sustained, selective, alternating, divided), reduced working memory, impulsivity, disinhibition, impaired initiation, and difficulty with planning and self-monitoring. The Rancho Los Amigos Levels of Cognitive Functioning (I–X) frame recovery stages; at low levels (III–IV, confused-agitated) treatment is environmental management and orientation, not table-top cognitive tasks. Dementia and cognitive-communication
In dementia, communication decline is progressive and follows the underlying pathology: Alzheimer's begins with episodic memory and anomia; frontotemporal dementia begins with behavior or language; Lewy body dementia adds fluctuating attention and visual hallucinations; vascular dementia progresses stepwise. Evidence-based SLP interventions include spaced retrieval training, errorless learning, external memory aids, Montessori- based activities, and caregiver/communication-partner training — restoration of memory is not a realistic goal.

- RIPA-2 (Ross Information Processing Assessment) for cognitive-linguistic domains.
- Cognitive Linguistic Quick Test (CLQT+) for attention, memory, executive function, language, visuospatial skills.
- MoCA and SLUMS as brief screens (screening only, not diagnostic).
- Behavioural Inattention Test for neglect; discourse sampling for pragmatics.
Treatment principles
Effective cognitive-communication treatment is functional and context-based: train in the environment where the behavior must occur, use metacognitive strategy instruction (goal- plan-do-review), build external supports (calendars, checklists, phone alarms), and train communication partners. Isolated computerized drills rarely generalize, and that limitation is a frequently tested point.
